Observation Rounds

Rounds that prove the patient was seen

Set the level and the rounds appear on their own. Each check is recorded when it happens, with where the patient was and how they were, and a check nobody made stays a gap.

Observation Rounds
What it does

Built into the day-to-day

  • General, every 30 minutes, every 15 minutes, constant and arms length
  • Rounds generated as soon as a level is set
  • Location and state recorded in a couple of taps
  • Overdue checks flagged while there is still time
  • Missed checks recorded as gaps, not filled in later
  • The level linked to the risk assessment behind it

Questions

Frequently asked

Can a missed check be filled in afterwards?
No. Once a check is missed it stays missed, and what happened goes in the nursing notes. A round that can be completed later proves nothing.
What about patients on constant observation?
Constant and arms length observation are recorded as levels without timed checks, because the patient is never out of sight.
