Seclusion & Restraint
A register that cannot be tidied up afterwards
An episode is examined later by people who were not there. The register records why it started, who authorised it, every review while it lasted and how it ended, and locks when it closes.
What it does
Built into the day-to-day
- Seclusion, manual, mechanical restraint and rapid tranquillisation
- A reason required before an episode can start
- Timed reviews with a prompt when one is due
- Whether anyone was injured, asked before it can close
- A debrief recorded after the episode ends
- Closed episodes locked, with later notes added as addenda

The forms and views behind it
The screens your staff actually use to run this, and what each one saves them.
Open episodes first
A running episode sits at the top with its next review, above the closed ones.
Part of the ward workspace
The register sits alongside rounds, risk and the legal register in the psychiatry menu.
Questions

Yes. The register has a printable view laid out for inspection.
What if something is remembered after an episode closes?
It is added as a dated addendum. The original entries stay exactly as they were written.
